Externship Requirements

Prior to starting on-site externships, you’ll need to complete requirements for health and safety required by your employer.

Due Date: Sunday October 16th

Please reach out to your coach if you have something preventing you from moving forward with these requirements.

This visit needs to take place before November 13th. The doctor must fill out the form here.
If you don’t already have a primary care provider, you can find one via zocdoc.com or freeclinics.com.
If this if your first shot, you will need to receive and upload the first shot by Oct 16th.
You can submit your COVID card or upload this as a part of your immunization records. If you are in Camden, you will need to also get the COVID booster as required by Cooper University Hospital.
Immunization records must show proof of the following vaccines:
Varicella/chicken pox (2 doses)
MMR (2 doses)
Hep B (3 doses)
TDap (1 dose)

Many of our externship sites require that anyone serving in an externship at their site has insurance to protect themselves.

Start to talk with your family and do research on your options to secure coverage. We understand that finding and paying for health insurance can be a challenging process. Please consider the following resources:
- Healthcare.gov: This is the open marketplace available through the Affordable Care Act. Open enrollment runs from Nov 1 - Jan 15th.
- State-based exchanges: This website lists the states that have their own insurance exchanges as well. Scroll down on the page to identify if your state has a program.
